Cardiac Rehabilitation

The Sutter Roseville Cardiac Rehabilitation program follows the standards set by the Center for Medicare and Medicaid Services, American Association of Cardiovascular and Pulmonary Rehabilitation, the California Society for Cardiac Rehabilitation and the American College of Sports Medicine.
At the Sutter Roseville Cardiac Rehabilitation Center, we offer our patients comprehensive cardiac diagnostic, rehabilitation, and preventive services. Our outpatient Cardiac Rehabilitation program provides medically supervised exercise and education to help patients return to a healthy life after a cardiac event. Our program consists of phase II and phase IV of the four phase program. All phases require a physician referral. (Getting a referral can be easy! Print out our physician referral form and have your doctor fill it out)
Phase II – Exercise & Education
The out-patient phase is a one to three month ECG monitored exercise and education course designed to build physical strength and emotional confidence. This experience will provide you with knowledge of your health issues and the tools to actively manage your health. A multidisciplinary staff of cardiac care RNs, Exercise Specialists, and a Dietician will give you the tools you need to live an active healthy lifestyle.
Phase VI - Prevention* Newly Expanded!
A comprehensive Maintenance and Primary Prevention Program of minimally supervised non-monitored exercise for individuals in stable physical and mental health who have a physician referral and meet one or both of the following qualifications:
- Graduated from Phase II of the Cardiac Rehab program
- Been diagnosed with one or more cardiovascular risk factor (uncontrolled blood pressure, high cholesterol, diabetes, elevated blood lipids, obesity, smoking, and physical inactivity).
*There is a minimal fee associated with this phase, it is a self-pay program not covered by insurances.
Cost of the Program
Since the program is medically prescribed and supervised, many health insurance companies, including Medi-Care cover the cost of the program. If assistance is needed in seeking approval from insurance companies, our staff will be glad to help.
